Decide which part of your identity needs to lead. If people first meet you through a tiny profile image, look for one recognizable graphic that can stand alone. If your practice name is what you want them to remember, start with a template where the lettering has presence. If you make social posts, offers, and client materials, notice the little motifs you could use again without repeating the full logo everywhere.

Put your two favorites side by side with your own name, then shrink them to the size of a phone profile image. Look at them at normal phone distance, not zoomed in. Keep the one whose name or main symbol you can still recognize. A design that only works enlarged will be hard to use in the places clients see most often.
